Registration Guidelines

  1. Registration is open to presenters and non-presenters, however, only undergraduate students are allowed to participate as a presenter.

  2. For Students and Lecturers -- Registration fee: RM90; With accommodation: RM150 (Including event T-shirt, meals, goody bag, certificate of participation, nametag)

  3. For Non-Students - Registration fee: RM90 (Without accommodation)

  4. Each participant and presenter is required to fill up the registration form in order to participate in this symposium.

  5. The registration form is in the form of Google Form. The link of each Google Form will direct to the registration form of each respective university.

  6. Please fill in the registration form according to university as listed below.

  7. The registration form must be filled completely and once only.

  8. All the registration must be done before 15 APRIL 2019, any registration after the deadline will not be entertained.

  9. Bank Islam 01-087-02-372812-0 (FATHIN AMIRA BINTI MOHD RAMLI), every university representative will responsible to collect each participant's payment and send to the given bank account number.

Insurance
The registration fee does not include participant’s insurance against accidents, sickness, cancellation, theft, property damage or loss. Participants are advised to take out adequate personal insurance.

Group payment
Every university representative will be held responsible to collect each participant’s registration fee. The payment must be transferred to the given bank account number and kindly send the transfer receipt along with necessary contact details (university representative) to 10mysimbiosofficial@gmail.com for record-keeping purposes. Payments received without such details cannot be identified and will be considered as void.

Confirmation
Each university representative will receive an email notification confirming the received payment and eventually state the amount outstanding after the completion of registration.


Final invoice
The final invoice will be provided to the university representative only upon request. Please contact us to ask for your receipt.

Refund policy
Fees refunds are not given to participants who are unable to attend the event. They may transfer their registration to another individual with prior notice to the university representative.
